Hello! Now that you’ve scheduled your appointment, it’s time to fill out your client forms.

* NOTE:

  • If you are a returning client and have already filled out forms on this platform in the past year, you do not need to fill out forms UNLESS you have scheduled another type of appointment (i.e. you’ve had a cosmetic tattoo appointment, but are now getting a body tattoo)

  • If you have a permanent jewelry appointment, you are not required to fill out forms!
